When you need to report an issue with your phone please provide the extension # in your report.  This helps us map your phone to a dedicated mac address in the system to run down any possible leads.

To assist in reporting the issue I've added a Phone Issue form to the ticket system that you can use.  While technically the personal information section already has a phone # option that should be used for how I can get a hold of you.  If your phone is down I can't contact you via that number.  So adding a separate extension field seemed the best solution.
